Parent Volunteer Background Check

All volunteers who will be supervising children are required to undergo a criminal background check through the Oregon Department of Education. Please fill out the Criminal History Verification form, then upload it here. 

Background checks are valid for two years. If you are unsure if you have a valid background check on file, please email Penelope Babst.
